WHAT IS SPONSORED RESIDENTIAL

Sponsor residential serves as a viable alternative to group homes, offering a more family-like environment. This model emphasizes on skill building, routine supports, general assistance and safety measures, all provided around the clock. The care is delivered in the homes of families or person (sponsors) in a family-like setting, under the oversight of a licensed provider from the Department of Behavioral Health and Developmental Services (DBHDS). The primary goal of the sponsor is to help individuals develop, maintain, or enhance essential self-help, social and adaptive skills. These supports are crucial for enabling individuals to live successfully within their homes and communities, promoting greater independence and integration into the community.

WHAT WE DO

We address the needs of individuals with ID/DD to live in a family-like setting while gaining independence and skills. These homes provide a safe, supportive environment where individuals can pursue their goals and stay connected to the community.

  • Individual must be at least 18 years of age
  • Have a primary diagnosis of a developmental disability and may have a secondary diagnosis of a mental illness
  • Have a Community Living Waiver (CL)
  • Complete an application
  • A physical with a negative PPD must be completed prior to admission
  • Intake Form
  • Schedule a home visit or tour
  • Submit most recent screenings

Monday – Friday 9:00am – 5:00pm
Saturday 8:00am – 12:00pm
